 This form is for submitting fundraising requests for any activity, sale, event, or campaign that uses the Booster Club’s nonprofit status or EIN. Requests must be submitted at least 30 days before the fundraiser start date, and a representative must be available to present the request at a meeting. No communication or promotion should occur until board approves the request.

 This form is for requesting reimbursement or payment of an approved invoice for items or services purchased for the school or Sport or Club. Requests must be submitted within 30 days of purchase and include receipts, proof of payment, or an invoice. Submitting a request does not guarantee reimbursement or payment, as all requests are subject to eligibility, available funds, and policy requirements.

 This form is for requesting grants that support school-wide programs or events benefiting all students. Requests must be submitted at least 30 days before funds are needed. Approval isn’t guaranteed, as funding depends on eligibility, available budget, and policy requirements.
