This form is for submitting fundraising requests for any activity, sale, event, or campaign that uses the Booster Club’s nonprofit status or EIN. Requests must be submitted at least 30 days before the fundraiser start date, and a representative must be available to present the request at a meeting. No communication or promotion should occur until the Board approves the request.
Important: All fundraiser proceeds must be associated with an approved Fundraising Request. If the Booster Club receives a check or payment that cannot be matched to an approved fundraising request, the funds will be deposited into the Booster Club General Fund.
Use this form for any request involving the payment, reimbursement, disbursement, or movement of GKHS Booster Club funds.
Programs requesting Booster Club funds are expected to remain active and in good standing, including participation in general membership meetings and maintaining appropriate financial records.
Every transaction must have a matching submitted form. If there is no form, no payment will be issued.
Reimbursement requests must be submitted within 30 days of purchase, and complete requests may take up to 14 business days to process.
Submission of a request does not guarantee payment. All requests are subject to available funds, program standing, required documentation, GKHS Booster Club bylaws and policies, Bethel School District requirements, WIAA rules and regulations, applicable laws, and compliance review.
This form is for requesting grants from the GKHS Booster Club General Fund to support school-wide programs, activities, or events that benefit GKHS students.
Grant requests must be submitted at least 30 days before funds are needed. A representative must attend a general membership meeting to present the request, answer questions, and provide any additional information needed for consideration. Submission of a grant request does not guarantee approval or funding.
Grant awards are subject to available General Fund resources, eligibility, membership approval, GKHS Booster Club bylaws and policies, Bethel School District requirements, WIAA rules and regulations, and applicable state and federal requirements.
Please do not make purchases or financial commitments based on anticipated grant funding until the grant has been formally approved.
